SISC Student Accident Coverage and Procedures

The college insurance is an accidental-injury policy providing secondary coverage. There is a 52-week incurring period and exclusion for any pre-existing condition. The purpose of this letter is to familiarize you with the college insurance procedures. In order for a claim to be considered the following procedures must be adhered to:

  1. Notify the Certified Athletic Trainer immediately following any school/athletics-related accidental injury. NOTE: All student athletes must coordinate their medical treatment with the college athletic trainer first.
  2. File a claim with your primary insurance carrier if outside medical treatment is necessary. The college insurance provides secondary coverage; therefore, it is essential that you submit a claim regarding your injury to your insurance first, if you have such primary insurance coverage.
  3. Forward medical bills promptly to your own primary carrier. If your insurance does not cover the bill, or you have no primary insurance, notify the PC Business Office (located in AC Building on PC Campus or call (559)791-2492) as soon as possible.  Do not ignore your medical bills; to do so might have a detrimental effect on your credit.
  4. Bring or send the explanation of benefits statement (EOB) from your primary insurance, if any, to the PC Business Office when you, or your parents, receive it. The college insurance requires a primary EOB, if you have primary coverage, for each bill you submit, and the bills must be itemized.
  5. Complete and return promptly any form requested for information to the Business Office. No medical bills can be processed for payment without an authorized claim form and verification of other insurance form. Claim forms are available in the Athletic Training Clinic or the PC Business Office.  Please provide the PC Business Office with the names of all medical facilities involved in the treatment of your injury.
