Your Pre-Participation Physical Exam must be completed by a licensed physician. The California Community College Athletic Association only recognizes PPEs provided by MDs or DOs (or his/her PA/FNP). – No Chiropractors!

The PPE History Form must be completed and printed prior to your physical appointment. Please hand it to the physician for review. The Physical Form page MUST BE completed by the clinician. Please ensure that the doctor reads over both pages prior to examining you and signs, dates, initials, and stamps the third page. Please bring the PPE History form AND physical to the Athletic Training Clinic.

PPE  must be on our form and will not be accepted without

- signature and initials of medical provider

- date of exam

- office stamp with name of provider (and name of MD PA/FNP practice under)
